DIRECTORY OF SENIOR HEALTH CARE FACILITIES

This directory is to be used for information and clarification in choosing the best possible senior housing and/or level of care available in the City of Santa Monica.

The reader will have to make a personal investigation of any facility or facilities listed herein as we do not recommend or endorse one facility over another, nor is an inclusion or omission of any facility or provider in this directory to be construed as a recommendation or lack of one by the commission or the City Council.

There are discharge planners at the acute care hospitals who can help determine the best placement or living arrangement if you cannot return home by yourself after a hospital stay or if you need other care. They will consult with you, your doctor, family, the social worker at the hospital, and various community agencies on your behalf.

2.  MEDICARE-MEDI-CAL
Most hospital stays are relatively brief. If a patient still needs skilled nursing care, the doctor will have the patient discharged from the acute care hospital to a nursing home.
Medicare Nursing Home benefits will continue for a limited number of days, after which a patient has to spend their own savings (as a private patient) down to a certain amount before they can qualify for Medi-Cal Nursing Home benefits. Then a patient may be transferred to another facility, perhaps even out of Santa Monica, if the first nursing home is not licensed for Medi-Cal.
Some nursing homes, even though they are licensed for Medi- Cal, will not accept new patients who are already on Medi-Cal. Not all nursing homes will take Medi-Cal patients even if they do accept Medicare. There is different licensing, certification, and reimbursement to the nursing home for each program and there are a limited number of beds in reserve. Some nursing homes will take private patients only.
Be sure to discuss these matters thoroughly with your doctor and the nursing home administrator.
 

3.  NURSING HOMES  
A Nursing Home is also known as a Skilled Nursing Facility, a Long-Term Care Facility, a Convalescent Hospital, and a Convalescent Home. It is licensed and monitored by the State of California's Department of Health Services, and is governed by Title 22 regulations.
The nursing home provides skilled nursing care for long- or short-term periods of recuperation or care for any age adult, usually elderly, with primarily physical conditions. You must be admitted by, and under the care of, a physician to be in a nursing home. There are fourteen of these facilities in Santa Monica:

4.  OMBUDSMAN PROGRAM  
Ombudsman is a state-mandated program of experienced professionals and volunteers who respond to problems in long-term care facilities, including Board and Care Homes and Nursing Homes. Volunteers are trained to visit on a regular basis (or on call) and  investigate, report, and/or negotiate and resolve problems on behalf of patients, families, staff, and community. 5.  PATIENTS' RIGHTS  
The Long Term Care, Health Safety and Security Act amended sections of Title 22 of the California Health and Safety Code and created new requirements concerning nursing home care in California.
The law became effective on January 1, 1974. It sets minimum standards of care in nursing homes and empowers the State to impose civil penalties upon substandard facilities. Section 72523 of Title 22 specified patients' rights. Each patient has the following rights:
1. To be fully informed of his/her rights and of all rules and regulations of the nursing home which govern patient conduct.
2. To be fully informed of the services available in the facility and of the charges for services.
3. To be fully informed by a physician of his/her medical condition;* to have the opportunity to participate in the planning of his/her medical treatment, and to refuse to
participate in experimental research.
4. To refuse treatment to the extent permitted by law and to be informed of the medical consequences of refusal.
5. To be transferred or discharged only for medical reasons, or for his/her welfare or that of other patients or for nonpayment of his/her stay and to be given reasonable advance notice.
6.  Be encouraged and assisted throughout his/her period of stay to exercise his/her rights as a patient and as a citizen, free from restraint, interference, coercion, discrimination or reprisal.
7. To manage his/her personal financial affairs, or to be given at least a quarterly accounting of financial transactions made on his/her behalf if the facility accepts this responsibility for him/her.
8. To be free from mental and physical abuse and from chemical and physical restraints except as authorized in writing by a physician for a specified and limited period of time, or when necessary to protect the patient from injury to himself or herself or
to others.
9. To  be assured of confidential treatment of his/her personal medical records, and to approve or refuse their release to any individual outside the facility.
10.  To be treated with consideration, respect and full recognition of his/her dignity and individuality, including privacy in treatment and care for his/her personal needs.
11. Not to be required to perform services for the facility that are not included in  his/her plan of care for therapeutic purposes.
12. To  associate and communicate privately with person(s) of his/her choice and to send and to receive personal mail unopened. *
13.  To meet with and participate in activities of social, religious, and community groups at his/her discretion. *
14.  To retain and use his/her personal clothing and  possessionsas space permits, unless to do so would infringe upon rights of other patients.*
15.  If  married, to be assured privacy for visits by his/her spouse;  and if both are patients in the facility, to be permitted to share a room. *
16. To have daily visiting hours established.
17. To have members of the clergy admitted at the request of the patient or person responsible at any time.
18. To allow relatives or persons responsible to visit critically ill patients at any time. *
19. To be allowed privacy for visits with family, friends, clergy, social workers, or for professional or business purposes.
20. To have reasonable access to telephones both to make and to receive confidential calls.
21. To use pharmacy of his/her choice.
* The patient's rights may be denied or limited for good cause only by a physician. Denial of such rights shall be documented by the attending physician in the patient's health record.

Senior Health and Peer Counseling is funded partially by government agencies: City of Santa Monica, Los Angeles County Area Agency on aging through the Older Americans Act 1965 as amended, and the Short-Doyle Plan of the Los Angeles County Department of Mental Health. Private sector support has been received from: W.M. Keck Foundation, W.K. Kellogg Foundation, National Medical Enterprises, Inc., Mark Taper Foundation, and U.S. Department of Education. Additional support comes from United Way and private donations.

On-going support group for adults 55 years and older.
Meets at Senior Health and Peer Counseling'sArizona location.

THE PURPOSE OF THIS GROUP IS TO:
     * BRING STROKEES TOGETHER TO ASSIST AND SUPPORT ONE ANOTHER
     * PROVIDE GROUP DISCUSSION TO HELP OVERCOME CHANGES AND HANDICAPS
     * ENCOURAGE AND SUPPORT STROKEES TO WORK TOWARDS SELF IMPROVEMENT

Elaine Yarus who will be the facilitator of this group is well trained in group leadership and has worked with stokees for 17 years.  She has had personal experience with stroke in her own family.

Dr. Brent Oldenburg, Ophthalmologist will provide the screening and answer your questions about common eye problems. A simple vision test will be given. No glass prescriptions will be written at this clinic.

All services are subject to a small sliding scale fee.

SERVICES ARE PROVIDED BY A LICENSED PODIATRIST

Because treatment is provided, a fee is charged.  The podiatrist accepts and will bill Medicare and private insurance, if the initial deductible has been met.  Assignment will be taken. For those persons not covered by insurance,or belonging to an HMO, a flat fee of $20 is charged.

HEARING TEST
for anyone 55 years and older

in the clinic at 2125 Arizona Avenue, Santa Monica

A trained audiometrist will evaluate your hearing to determine if any loss is present. If a loss is identified, you will be referred for more in-depth testing and if necessary a hearing
aid evaluation. Unfortunately, we do not repair hearing aids. If this is the problem, please return to the place you purchased them.
